Department Forms and Applications

Below are forms you may need to complete to make changes to your academic plan.

Please complete all forms in PDF format, adding any supplementary information via PDF as well. Submit documents as noted below, via email only.

Change of Major Form

Students must submit this fully completed Application Packet and a Change of Major form, with proper signatures and fully completed, when required to change their Program/Plan (Change of Major).

  • A Change of Major form will not be accepted without completion of the entire packet (Major Application, Program Planner and Change of Major Form).
  • The Department packet is available for download. Communication Studies Major Application AY19-20
  • Download a current Change of Major form.
  • Download the current Program Planner for Communication Studies.
  • Please note the complete packet, including the Change of Major form, is available in the Communication Studies Office.

Required Signatures: Student, Department Chair

Send PDF of full packet to Program Assistant.


Change of Minor Form
Students must submit this form at least one semester before graduation.
Required Signatures: Student, Department Chair

Send PDF of full packet to Program Assistant.


Change of Plan/Declare Track Form
Students must complete this form with the proper signatures in order to change their major or plan.
Required Signatures: Student, Department Chair

Send PDF of to Program Assistant.

To Declare Specialization: Fill out only the highlighted fields on the form and send PDF directly to Records & Registration.

The Chair’s signature is not needed when declaring your track/specialization.


Course Overload Request Form and Planner
Students must complete this form with the proper signatures in order to take more than 4.5 units in a semester.
Required Signatures: Student, Department Chair, Assistant Dean

Independent Study Form
Students must submit this form at the time of registration. A copy of the Independent Study or Mentored Research proposal must accompany this form.
Required Signatures: Student, Faculty Supervisor, Department Chair, Assistant Dean

Send PDF of full packet to Program Assistant once Faculty Supervisor has signed the PDF.


Interdisciplinary Concentration Application Form
Students must submit this form with the proper signatures in order to declare and interdisciplinary liberal learning concentration.
Required Signatures: Student, Concentration Advisor, Major Advisor

Send PDF of full packet to Program Assistant once Advisors’ signatures are completed.


Internship Enrollment Form
Students must submit this form at the time of registration. A copy of the Internship proposal must accompany this form.
Required Signatures: Student, Faculty Supervisor, Department Chair, Assistant Dean

Send PDF of full packet to Program Assistant once Faculty Supervisor has signed the PDF.


Program Planner
Students can complete this form to ensure that they are fulfilling the Communication Studies Major Requirements as well as the liberal learning, civic responsibilities, and potential minor requirements.
